If you have forgotten your username or password, complete the following steps to have Course Insite^® mail you your username and password:

1. On the Login screen, click the Forgot Your Password link to go to the Forgot Your Username or Password? page.

1. Complete as many of the following text boxes as possible:
   • First Name—Your first name.
   • Last Name—Your last name.
   • Address, City, State, Zip—The address you provided to Course Insite^®.
   • E-mail—The e-mail address you provided to Course Insite^®.
2. Click the Submit button to have Course Insite^® mail your user name and password. Course Insite^® will display a confirmation page.
